What is Chronic Disease?

Chronic diseases are long-term health conditions like high blood pressure or diabetes. To cope with these chronic conditions, people often have to make big changes in how they live each day.  If you or someone you love is living with a chronic disease, your physical and emotional health can be affected.  However, there is help and there is hope! 

People living with chronic health conditions are encouraged to take part in free classes in their communities. These classes offer tips for learning how to better control symptoms and be an active "partner" in the care of chronic conditions.  Healthy eating, exercise and positive attitudes are important in enhancing EVERYONE'S quality of life, particularly people with chronic health conditions. 

With a positive approach and support to take charge of your chronic health condition, you CAN lead a better life!
