Ambulatory Care Gold Standard Experience

Capital Health has six initial integrating services initiatives that will improve coordination of care across the health care spectrum. These integrating services initiatives are about building stronger connections between health services, people and providers to better support patients in their care journey and realize all the benefits of a regional health system. These initiatives include:

  • Ambulatory Care
  • Emergency Services System Capacity
  • Integrated Cancer Care
  • Bone and Joint Health
  • Neurosciences
  • Chronic Disease Management

In the area of Ambulatory Care, the initial focus is on two projects:

  • Gold Standard (Ideal) Patient Experience
  • Enterprise Scheduling and Ambulatory Electronic Medical Record (EMR) System — eCLINICIAN

To meet the goal of establishing high quality, patient-centred Ambulatory Care Services, Capital Health sought to develop principles and a framework for the Ambulatory Care Gold Standard Patient Experience. These principles and framework will enable Capital Health will enable physicians, clinicians and staff to consistently deliver the ideal patient care experience to all of their patients in ambulatory/outpatient settings.

The Ambulatory Care Gold Standard Patient Experience is expected to:

  • Enable high quality patient outcomes
  • Enable cost effective use of resources
  • Enable a high standard relative to patient safety
  • Deliver an environment which provides high patient and family/caregiver satisfaction
  • Be scalable to the regional level to facilitate effective management of patient across the continuum of care

The vision for the Ambulatory Care Gold Standard Patient Experience is Working Together for Your Health. To guide the delivery of this vision, six overarching principles—along with the related supporting principles, standards to measure these principles, and sample enablers to achieve these principles—were developed.
