Start Thinking About Setting Goals

Use this handout to start writing down the healthy lifestyle goals you want to reach.  How do you feel about making lifestyle changes today? On the line below, make a mark that shows how ready you are:

Now, write down your answers to the following questions:

  1. What are some lifestyle changes you feel are important to work on?
  2. What stops you from making these lifestyle changes?
  3. How can you get around this?
  4. Who helps you or supports you in your daily life?
  5. Can these people help you make lifestyle changes?
  6. If weight loss is a goal for you what is a realistic goal?

Goal Setting

Making changes to your lifestyle is easier when you make "SMART" goals.  Use the "SMART" guidelines below when you make goals.

Specific  - Decide what you are going to do and how to do it.
Measurable - Keep track of how you are doing.
Attainable - Pick something you think you can do.  Start with small changes.
Rewarding - Think of how good you will feel when you make small changes.
Time Frame - Give yourself a time limit to reach your goal.

Based on the "SMART" Goals mentioned above, think of a goal(s) you would like to work toward.  Write down the goal(s) and how you will do it.
