Making changes is easier when you are aware of your daily routine. Write down your food and activities to get you thinking about what works best for you, and what changes you want to make. Use this tool every day to help keep you on track and reach your goals.
How to use this tool:
- Write down all the foods and drinks you eat. Include the portion size, how the food was prepared and any added foods like sugar, cream, margarine, sauces and dressings.
- Write down all your activities. Include planned activities like going to the gym or walking, and activities of daily life like grocery shopping or house cleaning.
- Write down your mood, and other factors that affect your choices, like how busy you are, or whether you are at home, work or a restaurant.
- Include weekdays, weekends and holidays.
- Look at the sample below to help you fill out the Food and Activity Record.
